Classic Blue HMO Network Directory and NWM Medical Group Addendum
Below is the Chicago Park District Classic Blue HMO Directory, which includes all of the contracted providers, facilities, Medical Groups, and IPA's that are participating in this new network. Since the Classic Blue HMO directory does not include listings for two Medical Groups, which are Northwestern Medical Faculty Foundation (Medical Group #466) and Northwestern Health Care Corp (Medical Group #467), an addendum with these providers is also available below.
In the addendum, Medical Group #466 has been completed. Medical Group #467 is still in process. The Park District will post a revised, updated listing of this addendum in the near future.
